Description: Registered Nurse Case Manager - Providence Home Services Who we need! Providence Home Services is seeking a full time Registered Nurse Case Manager. The primary responsibilities include providing evaluation and nursing services on an intermittent basis in the patient's place of residence. The Case Manager may function independently and/Operating Room in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Washington State Nurse Practice Act, based on the nursing process model and concept of community health nursing. The function of the position actively incorporates the Mission and Vision of Providence Health Systems. Requirements: Education: Graduate of an accredited school of nursing. Bachelor's preferred. Experience: At least three years full time experience practicing as a registered nurse, with one year of recent experience (within last three years) clinical experience in a Medicare certified Homecare or Hospice agency/facility doing intermittent visits and case management. Licensure/Certifications/Registrations/Permits: Current Washington State Registered Nursing License; Current Washington State Driver's license and have access to daily use of an insured, reliable vehicle; Current CPR certification.
