Description: LPN Case Manager: Thurston County - Providence SoundHomeCare & Hospice Who we need! Providence SoundHomeCare and Hospice has an opportunity for an LPN Case Manager. This professional is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. Functions independently and/Operating Room in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Washington State Nurse Practice Act, based on the nursing process model and concept of community health nursing. The function of the position actively incorporates the Mission and Vision of Providence Health Systems. Requirements: Education: Graduate of an accredited Nursing program. Experience: At least three years full time experience practicing as a registered nurse with one year of recent experience (within last three years) clinical experience in a Medicare certified Homecare or Hospice agency/facility doing intermittent visits and case management Licenses/Certifications: Current Washington State LPN License; Current Washington State Driver's license; Current CPR certification; and have access to daily use of an insured, reliable vehicle.
